ALL on-campus MCCN students already have 24/7 badge access to enter the MCHS Library.
You DO NOT need to complete this form!

Access is provided after hours to faculty and medical staff with badge authorization. On-line MCCN students in the Columbus, Ohio area may request 24/7 badge entry to the MCHS Library by completing the form below.
